एम्स के विशेषज्ञ चिकित्सकों के अनुभव और टीम वर्क की कौशलता की यह एक मिसाल है। एक साल से अधिक समय से फीडिंग पाईप से भोजन ग्रहण कर रही जो महिला अपनी बीमारी को नियति मानकर जीवन की आस खोने लगी थी, डाॅक्टरों की टीम ने सर्जरी द्वारा उसकी नयी आहार नली बना दी। आम लोगों की तरह मुंह से भोजन शुरू हुआ तो मानों महिला का जीवन वापस लौट आया। सर्जरी के बाद महिला अब आम लोगों की तरह भोजन ग्रहण कर रही है और अपने घर पर रहते हुए पूरी तरह स्वस्थ है।
एसिड युक्त टाॅयलेट क्लीनर पेट में चले जाने के कारण मुरादाबाद की एक 24 वर्षीय महिला की आहार नली पूरी तरह से जल गयी थी। इस वजह से वह पिछले 13 महीनों से फीडिंग ट्यूब के जरिए ही तरल भोजन पर निर्भर थी और मुंह से कुछ भी खाने-पीने में असमर्थ थी। इस दौरान उसका जीवन एक ट्यूब (फीडिंगजेजुनोस्टॉमी) के माध्यम से तरल आहार पर चल रहा था। चिकित्सीय भाषा में पेट में तरल भोज्य पदार्थ पहुंचाने की यह एक ऐसी व्यवस्था है जिससे ट्यूब नली के माध्यम से भोजन को सीधे पेट की छोटी आंत में पहुंचा दिया जाता है। एम्स आने से पहले महिला ने कई अस्पतालों से इलाज भी करवाया लेकिन कई अस्पतालों द्वारा एंडोस्कोपी करने के बाद भी उसकी भोजन नली में आयी रुकावट दूर नहीं हो पाई। आखिर में एम्स पंहुचने पर जांचों के उपरान्त सर्जिकल गैस्ट्रोएंटरोलॉजी विभाग के डाॅक्टरों ने रोगी की बड़ी आंत के एक हिस्से से नई आहार नली (इसोफेगस) बनाने का निर्णय लिया। सर्जिकल गैस्ट्रोएंटरोलाॅजी विभाग के हेड व सर्जरी करने वाले शल्य चिकित्सक डाॅ. लोकेश अरोड़ा ने इस बारे में बताया कि ‘‘कोलोनिक पुल-अप‘‘ नामक इस सर्जरी की प्रक्रिया में आंत का हिस्सा पेट से होते हुए छाती के रास्ते गले तक खींचा गया। जोखिम भरी इस सर्जरी में लगभग 7 घंटे का समय लगा और विभिन्न विभागों की संयुक्त टीम ने मिलकर इसे सफल बनाया। संस्थान की कार्यकारी निदेशक प्रोफेसर (डॉक्टर) मीनू सिंह और चिकित्सा अधीक्षक प्रो. बी. सत्याश्री ने इस उपलब्धि पर सर्जरी करने वाली टीम के कार्यों की प्रशंसा की है।
चुनौतियां
डाॅ. लोकेश अरोड़ा ने बताया कि इस सर्जरी की सबसे बड़ी चुनौती आहार नली के पास स्थित वॉयस बॉक्स को सुरक्षित रखना था। ऐसे में थोड़ी सी भी चूक होती तो स्वरयंत्र को स्थायी नुकसान हो सकता था और महिला की हमेशा के लिए आवाज जा सकती थी। इसलिए अलग-अलग विभागों के विशेषज्ञ चिकित्सकों की टीम गठित की गयी और टीम की गहन निगरानी की वजह से यह सर्जरी पूरी तरह सफल रही।
टीम में शामिल डाॅक्टर्स-
डॉ. लोकेश अरोड़ा (विभागाध्यक्ष, सर्जिकल गैस्ट्रोएंटरोलॉजी) के अलावा इस टीम में इसी विभाग की डाॅ. सुनीता सुमन, डॉ. नीरज यादव, डॉ. विनय, डॉ. अजहर, डॉ. शुभम, डॉ. अमन, ईएनटी सर्जन डॉ. अमित त्यागी, एनेस्थीसिया विशेषज्ञ डॉ. संजय अग्रवाल व नर्सिंग ऑफिसर दीप, मनीष, सीमा और रितेश आदि शामिल रहे।
ऐसे हुई रिकवरी-
सर्जरी के बाद रोगी को 5 दिन तक सीसीयू में रखा गया। जनरल वार्ड में शिफ्ट करने के बाद रोगी ने 8वें दिन से मुंह से भोजन करना शुरू कर दिया था और 15वें दिन उसे अस्पताल से छुट्टी दे दी गयी। हालांकि यह सर्जरी जनवरी माह में हो चुकी थी, लेकिन पिछले 4 महीनों तक डाॅक्टर्स नियमिततौर पर फोन द्वारा और फॉलोअप हेतु बुलाकर उसके स्वास्थ्य की माॅनेटेरिंग कर रहे थे। चिकित्सकों के अनुसार सम्पूर्ण भोजन करने से अब उसका 10 किलोग्राम वजन बढ़ गया है और वह सामान्य जीवन जीते हुए पूरी तरह स्वस्थ है।
